Domestic Partnership Registry
BACKGROUND
The Board of Commissioners adopted Ordinance No. 2013-34 on December 9, 2013 establishing a Domestic Partnership Registry in the City of North Port.
REGISTRATION
To register, partners must:
1. Complete an Affidavit for Domestic Partnership Registration. The form must be signed in the presence of a Notary Public in the Clerk’s office; and
2. Pay a filing fee; and
3. Each partner to present identification; and
4. Apply in person, together (by appointment only) at the City Clerk office:

Registering requires both parties to be present at the time the application is filed. Upon completion the registrants will receive a Certificate of Registration and a photo identification card.

FEES
Fees may be paid by cash, credit card, check, or money order payable to the City of North Port.

RECORDS

AMENDMENT or TERMINATION
An Amendment to Certificate is filed for a change in legal name or address. The Amendment must be signed by both partners in the presence of a Notary Public in the Clerk’s office.
A Termination of Domestic Partnership may be filed by one or both partners. The Declaration of Termination must be signed in the presence of a Notary Public in the Clerk’s office.
Application for an Amendment or Termination require a filing fee.
